Ball Below Your Feet - Choke down on the club to encourage a more flexed position in your set up.  This will help you make a swing that allows you to get solid contact at impact.  In the backswing, it is critical to maintain the posture you started with at address.  From here, swing through the ball maintaining your posture as best you can into the follow through.  Be sure to take a club that you can swing at a speed that you can easily control throughout the swing.  Most shots hit from this lie have a tendency to be miss-hit when you lose your posture.  I cannot stress enough the importance of staying flexed through out the swing.
Ball Above Your Feet - Notice how my body is standing more erect than when the ball was below my feet.  From here, my backswing motion is more rounded producing a flatter swing plane as a result.  It is important to note that this swing motion produces a shallower strike on the ball and encourages a right to left ball flight.  With that said, it is important to aim slightly right of your intended target.  Notice how my follow through is more complete than when the ball was below my feet.  This results from the different postures used on these shots along with the variations in the swing path.
